The mission of CustomCare MD is to restore the depth and efficacy of medicine by placing the physician-patient relationship at the center of our practice. We have created a clinical environment that rejects the transactional, high-volume model of conventional healthcare.
Exceptional care requires deep understanding, which can only be achieved through time and continuity. Our model provides the necessary space to explore complex health concerns, identify links between lifestyle and health, and foster the trust required for unhurried, substantive dialogue. This clinical and relational environment is the foundation of our practice. Success is not measured by patient volume, but by the quality of our clinical relationships and the meaningful impact we have on our members’ long-term health.
The vision for CustomCare MD originated from a clear need for a better healthcare model. In 2002, while practicing in a traditional high-volume medical group, Dr. Rick Tomm observed that patients were seeking a more accessible and attentive physician relationship, a model then known as “concierge medicine.” This approach, virtually unknown in Northeast Ohio at the time, aligned with his core clinical values.
After attending a conference by the American Academy of Concierge Physicians, he recognized that this higher standard of care could not be an addition to a traditional practice—it required a new model built from the ground up. Dr. Tomm subsequently launched one of the first concierge practices in the region, establishing a new standard for primary care in Northeast Ohio. For two decades, our practice has been committed to creating enduring physician-patient partnerships that prioritize thoughtful, relationship-based medicine.
